The below document requires notarization. Notary publics are available at banks, credit unions, shipping stores, office supply retailers, money transfer stores and more. DePelchin does have a notary public available, however, an appointment must be made 24 hours in advance and the appointment is not set until a confirmation email is received.
In addition to the above documents, a copy of your driver license and social security card is required. Documents may be emailed to Lauren Brezger at lbrezger@depelchin.org or dropped off at DePelchin's Memorial campus's front desk to Lauren's attention (4950 Memorial Drive, Houston, TX 77007). Our hours of operation are Monday through Friday, 8am to 5pm.
